Job Summary
Conduct longitudinal care for patients with high-risk chronic conditions by performing health assessments, medication reconciliation, and motivational interviewing. Provide patient education, create referrals to agencies, and coordinate transitions from inpatient to outpatient settings through contingency plans and post-discharge support. Collaborate with multidisciplinary teams including physicians and nurses to establish care plans, while documenting accurately in electronic medical records and participating in daily huddles. Maintain continued competence in evidence-based practices and perform assigned clinical tasks within the RN scope. This role supports WellMed's mission to improve care for seniors across Texas and Florida.
Required Qualifications
- Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience
- Registered Nurse with an active and unrestricted license to practice in the state of employment
- Current BLS certification
- 2+ years of experience in a physician's office, clinical or hospital setting
- Experience interacting productively with individuals and with multidisciplinary teams
- Knowledge of chronic diseases, especially CO PD / asthma, diabetes, CHF and IHD
- Proficient computer skills to work efficiently with electronic medical records
- Must be available for Corpus Christi, TX
- Candidates are required to pass a drug test before beginning employment
