Job Summary
Support patients with high-risk health conditions by identifying needs, performing assessments, and conducting medication reconciliation. Deliver patient education, facilitate motivational interviewing, and create self-management goal plans to empower individuals. Coordinate transitions from inpatient to outpatient settings by establishing contingency plans, managing prescriptions, and arranging DME appointments. Collaborate with multidisciplinary teams including physicians and nurses to update care plans and participate in daily huddles. Maintain accurate documentation in electronic medical records and prepare reports for weekly meetings.
Required Qualifications
- Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience
- Registered Nurse with an active and unrestricted license to practice in the state of employment
- Current BLS certification
- 2+ years of experience in a physician's office, clinical or hospital setting
- Experience interacting productively with individuals and with multidisciplinary teams
- Knowledge of chronic diseases, especially CO PD / asthma, diabetes, CHF and IHD
- Proficient computer skills to work efficiently with electronic medical records
- Must be available for Corpus Christi, TX
- Must be able to lift 50 lbs
- Must pass a drug test
