RN Health Care Supervisor - Day Habilitation

Coordinate medical services with each member's primary physician and oversee all health-care services provided to program members at multiple sites. Participate in interdisciplinary evaluations, obtain treatment plan approvals, and safeguard consumer dignity while documenting recommendations and advising staff of medical concerns. Provide direct supervision to Licensed Practical Nurses for minimum six hours weekly, manage medication reviews, and monitor vital signs and weights. Screen admission applications, develop nursing care plans, and educate consumers on hygiene and health concerns. Act as a member of the Interdisciplinary Team, attend DHSP meetings, and provide in-service training on universal precautions and bloodborne pathogens. Maintain consumer files, emergency authorizations, and CPR/AED certifications while adhering to all medical regulations and accreditation requirements.
