Job Summary
Supervise and support Certified Nursing Assistants (CNAs) under the direction of the Director of Nursing while collaborating with the interdisciplinary team to assess, plan, implement, and evaluate individualized care plans. Ensure initial baseline and periodic comprehensive assessments are completed on time, prepare for and facilitate physician rounds by flagging concerns, and initiate consultations or referrals based on resident needs. Perform administrative tasks including medical documentation, evaluations, and reports, deliver nursing care within scope of practice, and monitor residents for changes in condition such as confusion, fever, or mobility loss. Notify providers as needed and perform other duties as assigned by the Director of Nursing.
Required Qualifications
- Active RN license in good standing in the state of practice

- Experience in a skilled nursing or long-term care setting
