Job Summary
Promote wellness and improve outcomes for patients with chronic conditions by providing support, education, and assistance in disease prevention and maintenance. Develop individual health plans with patients and families to facilitate self-management, while coordinating care and follow-up for those receiving outpatient services. Collect and document assessment data, create patient Plans of Care, and evaluate progress toward desired outcomes. Administer medications per standards, verify care consistency, and provide effective discharge planning. Collaborate with interdisciplinary teams and physician staff to implement evidence-based nursing practices and report system variances.
Required Qualifications
- Current University of Arkansas System employees, including student employees and graduate assistants
- Proof of legal authority to work in the United States on the first day of employment
- Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
- Valid RN License
- Minimum of 1 year Registered Nursing experience
- Annual TB Screening
- Criminal Background Check
- Substance Abuse Testing
