Job Summary

Manage palliative care cases and coordinate services for patients and caregivers in a fast-paced hospice environment. Assess patients with advancing chronic or life-limiting illnesses, interpret clinical data, and implement appropriate treatment interventions and medication management. Utilize computer systems to track care plans, write reports, and communicate effectively with clients, staff, and the community while handling multiple projects simultaneously. Maintain a positive work attitude in high-stress situations, ensuring the focus remains on honoring life and supporting the journey from serious illness through end-of-life care.

Required Qualifications

  • Graduate of an accredited school of nursing in the State of California as a Registered Nurse
  • Three years of recent clinical experience
  • Computer skills
  • Ability to learn and assimilate the clinical computer program system
  • Knowledge of advancing chronic or life-limiting illness/conditions, signs/symptoms, treatment interventions, and medication for palliative care
  • Ability to handle multiple projects and tasks simultaneously
  • Uses time and resources efficiently to accomplish work objectives
  • Ability to work in a high-stress environment or with participants who are under stress
  • Maintain a positive work attitude while working with customers/participants, staff, and the community
  • Excellent verbal and written communication skills
  • Ability to analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations
  • Ability to write reports
  • Ability to effectively present information and respond to questions from groups or managers, clients, customers, and the general public
  • Demonstrated ability to write in a clear, understandable, and concise manner
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
  • Ability to compute rate, ratio, and percent
  • Ability to apply common sense understanding to carry out instructions furnished in written or oral form
  • Ability to deal with problems involving several concrete variables in standardized situations
  • Possess reliable transportation
  • Have a valid California driver's license
  • Have automobile insurance which meets State standards
  • Possess a license to practice as a registered nurse in the State of California
  • Must be able to drive distances including freeways and mountains when necessary
