Job Summary
RN Field Supervisor (Part-Time) coordinates and manages patient care plans and service delivery for clients in a fast-paced setting. Responsibilities include developing and implementing written Care Plans with client/family approval, ensuring Care Plans are reviewed and updated on-site, confirming Care Plan adherence by caregivers, maintaining patient records, and communicating observations to the supervisor. Qualifications require a Bachelor’s Degree in nursing or related human-services field (or 2 years of geriatrics/care management experience), active RN license in the employment state, and First Aid/CPR certification, plus a valid driver’s license. Flexible work-from-home options are available, with Iselin, NJ as a stated location.
Required Qualifications
- Bachelor’s Degree in nursing, social work, gerontology, psychology, health and human services, or other related field of human services OR 2 years experience in geriatrics and/or care management
- Currently licensed Registered Nurse (RN) in state of employment
- First aid/CPR certification required
- Valid driver’s license
