RN Field Case Mgr. (PRN Available!) - Southwest Territory

RN Field Case Manager to coordinate and manage hospice patient care in the El Campo service area. Responsibilities include coordinating care with providers (RN, LPN/LVN) and other team members, maintaining case documentation and notes per state regulations, developing and maintaining individualized care plans based on clinical assessment and family needs, communicating with admitting physicians or psychiatrists, conducting home visits, ensuring appropriate follow-up and referrals, coordinating with community agencies, providing training to staff on client management and referral processes, and maintaining case files in adherence with agency policies and procedures. Experience as a Registered Nurse and/or LPN/LVN and as a Case Manager is preferred.
