RN Field Case Manager - Telephonic WA State ONLY

Job Summary
Assess member health status and establish goals to meet identified care needs within the scope of licensure. Plan, implement, and evaluate responses to the plan of care while collaborating with multidisciplinary teams to address medical, functional, and social barriers. Work closely with mental health clinicians to bridge gaps between physical and mental health, consult with PCPs and specialists, and assess needs for community resources to make appropriate referrals. Facilitate patient transitions between health care settings and provide counseling and education on health maintenance and disease prevention. Completely and accurately document all interactions in the electronic medical record, verify patient understanding of the plan of care, and actively participate in organizational quality initiatives and team meetings. Maintain essential credentials, licensure, and CEUs. Full-time remote work in Washington state with a salary range of $91,500 to $137,300 annually.
Required Qualifications
- Current unrestricted licensure as RN in Washington
- 2+ years of experience as an RN
- Experience in assessing the medical needs of patients with complex behavioral, social and/or functional needs
- Computer skills, including us of Electronic Medical Records
- Proven ability to work with diverse care teams in a variety of settings including non-clinical settings (primarily patient homes)
- Access to reliable home internet
- Access to home office to maintain HIPAA privacy
- Must adhere to UnitedHealth Group's Telecommuter Policy
- Candidates are required to pass a drug test before beginning employment
Desired Qualifications
- Case management certification
- 6+ months Case Management experience
- Home health experience
- Geriatric experience
- Proven excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families
