Job Summary
Assess health status of members within licensure scope and establish goals to meet identified care needs. Plan, implement, and evaluate responses to the plan of care while collaborating with multidisciplinary teams to address medical, functional, and social barriers. Bridge the gap between mental and physical health by consulting with PCPs and specialists, assessing community resource needs, and facilitating patient transitions between settings. Complete accurate documentation in electronic medical records and provide counseling regarding health maintenance and condition trajectory. Actively participate in organizational quality initiatives and collaborative team meetings to optimize care delivery efficiency.
Required Qualifications
- Current unrestricted licensure as RN in Washington
- 2+ years of experience as an RN
- Experience in assessing the medical needs of patients with complex behavioral, social and/or functional needs
- Computer skills, including us of Electronic Medical Records
- Proven ability to work with diverse care teams in a variety of settings including non-clinical settings (primarily patient homes)
- Access to reliable home internet
- Access to home office to maintain HIPAA privacy
- Must adhere to UnitedHealth Group's Telecommuter Policy
- Candidates are required to pass a drug test before beginning employment
Desired Qualifications
- Case management certification
- 6+ months Case Management experience
- Home health experience
- Geriatric experience
- Proven excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families
