RN Field Case Manager - Telephonic WA State ONLY

Assess the health status of members, establish goals, and plan responses to the care model while collaborating with multidisciplinary teams to address medical, functional, and social barriers. Bridge the gap between mental and physical health by consulting with primary care physicians and specialists, then assess needs for community resources and facilitate transitions between care settings. Complete and accurately document all interactions in the electronic medical record, providing counseling and education on health maintenance and follow-up recommendations. Verify patient understanding of the condition and plan of care, actively participate in quality initiatives, and attend collaborative team meetings to optimize care delivery efficiency. Maintain current unrestricted licensure and credentials, including CEUs, while adhering to the Telecommuter Policy for remote work in Washington.
