Job Summary
Assess patient health status, establish care goals, and plan responses within the model of care. Collaborate with multidisciplinary teams and mental health clinicians to address medical, functional, and social barriers. Consult with primary care physicians and specialists, facilitate transitions between settings, and assess needs for community resources. Provide counseling and education on health maintenance, disease prevention, and condition trajectory while verifying patient understanding of the plan. Document all activities accurately in the electronic medical record and participate in quality initiatives and team meetings. Maintain current licensure, certification, and continuing education. This remote role supports Optum's integrated care model for patients in Washington, requiring reliable home internet and a private office space.
Required Qualifications
- Current unrestricted licensure as RN in Washington
- 2+ years of experience as an RN
- Experience in assessing the medical needs of patients with complex behavioral, social and/or functional needs
- Computer skills, including us of Electronic Medical Records
- Proven ability to work with diverse care teams in a variety of settings including non-clinical settings (primarily patient homes)
- Access to reliable home internet
- Access to home office to maintain HIPAA privacy
- Must adhere to UnitedHealth Group's Telecommuter Policy
- Candidates are required to pass a drug test before beginning employment
Desired Qualifications
- Case management certification
- 6+ months Case Management experience
- Home health experience
- Geriatric experience
- Proven excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families
