RN Field Case Manager- Snohomish and Kings Counties, WA

Visit members in their homes to assess current health status, identify treatment gaps, and provide patient education for self-management. Make referrals to outside sources and support members with condition education, medication reviews, and connections to resources like Home Health Aides or Meals on Wheels. Deliver a complete continuum of quality care through close communication via in-person home visits or phone interactions. This high-volume, customer service environment requires efficient, thorough handling of members over the phone with strong computer navigation skills. You must be patient-focused and adaptable to changes. A current, unrestricted RN license in Washington, Basic Life Support certification, and one year of clinical experience in home care are required. Residency in or near Seattle, WA, along with a driver's license and reliable transportation, is necessary.
