Job Summary
Conduct home visits to assess member health status, identify treatment gaps, and provide education on self-management, medication reviews, and resource connections. Make referrals to outside sources like Home Health Aides or Meals on Wheels while maintaining close communication via in-person or phone interactions to ensure a complete continuum of care. Navigate high-volume customer service environments efficiently, utilizing strong computer and software skills to support members with condition education and resource access.
Required Qualifications
- Current, unrestricted RN license in the state of Washington
- Certified in Basic Life Support, or ability to obtain by start date
- 1+ years of clinical experience
- Experience in home care/home visits
- Reside in or near Seattle, WA
- Driver's license and access to reliable transportation
- Must be able to lift 50 lbs
Desired Qualifications
- BSN
- Certified Case Manager (CCM)
- Case management experience
- Experience in discharge planning
- Experience in utilization review, concurrent review or risk management
- Experience in a telephonic role
- Experience working with MS Word, Excel and Outlook
- Background in managed care
- Bilingual in English/Spanish
