RN Field Case Manager- Snohomish and Kings Counties, WA

Visit members in their homes to assess current health status, identify treatment plan gaps, and provide patient education for self-management. Make referrals to outside sources and support members with condition education, medication reviews, and connections to resources like Home Health Aides or Meals on Wheels. Deliver a complete continuum of quality care through close communication via in-person home visits or phone interactions. This high-volume, customer service environment requires efficiency and strong computer navigation skills. You must be patient-focused and adaptable to changes while managing a full-time schedule from 8:00 a.m. to 5:00 p.m. PST. As part of the Optum Care at Home team, you help patients navigate the healthcare system to stay well at home.
