Job Summary
Visit members in their homes to assess current health status, identify gaps in treatment plans, and provide patient education for self-management. Make referrals to outside sources and support members with condition education, medication reviews, and connections to resources like Home Health Aides or Meals on Wheels. Deliver a complete continuum of quality care through close communication via in-person home visits or phone interactions. Work in a high-volume, customer service environment requiring efficiency, thoroughness, and strong computer navigation skills. This field-based position operates Monday through Friday, 8:00 a.m. to 5:00 p.m. PST, with hours that may vary based on business needs.
Required Qualifications
- Current, unrestricted RN license in the state of Washington
- Certified in Basic Life Support, or ability to obtain by start date
- 1+ years of clinical experience
- Experience in home care/home visits
- Reside in or near Seattle, WA
- Driver's license and access to reliable transportation
- Must be able to lift 50 lbs
Desired Qualifications
- BSN
- Certified Case Manager (CCM)
- Case management experience
- Experience in discharge planning
- Experience in utilization review, concurrent review or risk management
- Experience in a telephonic role
- Experience working with MS Word, Excel and Outlook
- Background in managed care
- Bilingual in English/Spanish
