Job Summary
Assess patient eligibility and pain management needs, develop individualized palliative care plans, and evaluate caregiver capabilities for home health patients in the NW Area. Manage symptoms and address physical, functional, and psycho-social impacts of serious illness or terminal diagnoses while identifying communication patterns. Apply specific criteria for admission and re-certification to establish appropriate levels of care. Requires an active Arizona RN license, CPR clearance, and minimum one year of recent professional nursing experience. Weekend on-call is not required.
Required Qualifications
- Graduate of an approved school of professional nursing
- Current, active license in the state of Arizona Board of Nursing, pursuant to ARS Title 32, Chapter 15 including fingerprint checks
- Current CPR, health clearance, TB clearance
- Minimum of one year of professional nursing experience within the last 3 years
