Job Summary
RN Field Case Manager role focusing on face-to-face advocacy and care coordination for injured workers; facilitate progressive medical treatment and timely return-to-work; serve as liaison among workers, families, caregivers, and medical providers; identify appropriate providers and services across the continuum to achieve cost-effective, optimal care; frequent travel required with a home-based office; remote work environment that enables in-person interactions; requires nursing licensure (RN) or a health/human services bachelor's degree with CCM/CDMS/CRC certifications; strong communication, problem-solving, and analytical skills; drive clinical and rehabilitation efforts within a supportive, value-driven culture.
