Rn Field Case Manager

Provide advocate and coordinate care for injured workers, families and caregivers, guiding them through recovery and facilitating timely return-to-work. Serve as a liaison among injured workers, medical providers, and employers, identify appropriate providers and facilities, and ensure resources are used in a timely and cost-effective manner. Utilize medical/clinical or rehabilitation knowledge to manage complex medical conditions and treatment planning. Requires frequent travel with a home-based office; role emphasizes face-to-face interaction with injured workers and medical professionals, advocacy, communication, education, and service facilitation. RN licensure preferred (or bachelor’s degree in health/human services with CCM, CDMS, or CRC); valid driver’s license and reliable internet required. Strong judgment, problem-solving, multitasking, and ability to meet deadlines, with accommodations as appropriate.
