Job Summary
RN Field Case Manager responsible for advocating, coordinating, and facilitating medical care and return-to-work for injured workers. Travel to meet with patients and providers, while maintaining a home-based office and ensuring cost-effective, timely care. Key duties include acting as patient advocate, identifying appropriate providers, coordinating services across the continuum, educating patients and families, and communicating with all involved parties to optimize treatment outcomes. Requires RN licensure, valid driver's license, ability to travel frequently, and access to high-speed internet. This role covers the Chicago, IL/Northwest Indiana region and requires living in the area.
Required Qualifications
