Job Summary
The RN Field Case Manager will conduct home health visits including SOC, ROC, recertification, and discharge visits, ensuring compliance with state regulations. Responsibilities include completing OASIS assessments, developing individualized Plans of Care, providing skilled nursing care, coordinating care with physicians, and educating patients and caregivers. Candidates must have a valid Texas RN license and prior home health care experience, along with reliable transportation.
Required Qualifications
- Must currently be licensed in the state of Texas
- At least 2 year nursing experience
- Prior home health care experience
- Reliable Transportation & Car insurance
- Ability to pass background check & have valid CPR
- English language proficiency
- Geriatrics experience: 2 years (Required)
- Home health experience: 2 years (Required)
Desired Qualifications
- IV infusion experience
- Experience with Oasis E and use of Wellsky/Kinnser
- Knowledge of ICD-10 codes
- Certified Registered Nurse Infusion
- Wound Vac Certification
- CPR/BLS Certification
