Job Summary
Oversee the clinical care of an assigned caseload in in-home nursing, assess client needs, develop and adjust personalized care plans, provide clinical supervision and direction to caregivers and nurses, collaborate with families and healthcare professionals to align daily care with goals, maintain accurate and compliant documentation, educate clients and families on care needs, identify caregiver training opportunities, support hiring and supervision of nursing staff, and participate in on-call rotation to provide 24/7 client support.
Required Qualifications
- Graduate of an accredited nursing program
- Active, unrestricted nursing license
- CPR certification (instructor certification preferred)
- At least 2 years of nursing experience, with home health, rehabilitation or case management strongly preferred
