Job Summary
Oversee the clinical care of an assigned caseload in in-home health, ensure optimal outcomes and regulatory compliance, assess client needs and develop personalized care plans, and provide clinical leadership to caregivers and nurses. Collaborate with families and healthcare professionals to align care with goals, maintain accurate documentation, educate clients and staff, identify caregiver training opportunities, support hiring and supervision of nursing staff, and participate in on-call rotations with local travel as needed.
Required Qualifications
- Graduate of an accredited nursing program
- Active, unrestricted nursing license
- CPR certification (instructor certification preferred)
- At least 2 years of nursing experience (home health, rehabilitation or case management strongly preferred)
- Strong clinical assessment, critical thinking and problem-solving skills
- Excellent communication and collaboration across clients, families, caregivers and medical professionals
- Compassionate, professional presence; ability to thrive in fast-paced environments
- Flexibility to travel locally on short notice
- BSN preferred
