Job Summary
In this community nursing role, provide in-home care to patients across designated areas, performing assessments, executing and monitoring care plans, and maintaining patient records. Travel to patient residences within the service area, engage patients to build rapport, and participate in on-call coverage after hours to ensure patient safety. Required Qualifications
- Registration with the CNO as a Registered Nurse in good standing
