Provide physical and psych-social support to residents and families while performing all clinical duties in accordance with state nurse practice act and nursing standards. Monitor clinical conditions to identify status changes, maintain infection prevention practices, administer medications, and perform treatments per orders. Establish resident care goals, teach self-care skills, and develop a comprehensive Holistic plan of care in collaboration with the interdisciplinary team. Ensure documentation is current, accurate, and timely, and participate in care conferences as requested. Full-time evening shift (3:00PM - 11:30PM) available. Minimum 6 months RN experience required with active NJ license and CPR certification. Preferred experience includes caring for seniors with cognitive impairment and familiarity with electronic medical records.
