Job Summary
Assess patient needs, coordinate referrals, and manage communication with patients, families, and the healthcare team to access services through Indian Health Services, Medicare, and Medicaid. Administer therapeutic measures, ensure accurate EHR documentation, and oversee team performance while implementing clinic policies and regulatory compliance. Lead population health initiatives, participate in grant development, and resolve conflicts to improve quality outcomes for the Elder population. Maintain professional knowledge through continuing education and collaborate with community resources to support continuous care.
Required Qualifications
- Graduate of an approved school of nursing
- Bachelor of Science in Nursing
- Current licensure as a Registered Nurse (RN) in the State of Oklahoma or a current multistate license from a Nurse Licensure Compact (eNLC) member state in good standing
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) issued by the American Heart Association
- Minimum three years (3) of experience as a practicing professional nurse (RN), including experience in performance management and daily oversight of patient care
- Must align with OKCIC vision, mission and core values
- Due to the safety-sensitive nature of this position, medical marijuana license holders who test positive for marijuana will not be qualified for this position
Desired Qualifications
- Experience in tracking, trending, and analyzing clinical performance data
- Experience working with older adults or in geriatric, senior care, chronic care management, case management, or care coordination
- Experience in performance management and daily oversight of patient care
