Job Summary
Full-Time Home Care Educator to represent and educate Case Managers in the field for Wellbound Certified Home Health Agency. Travel locally is required. Responsibilities include working with Case Managers to identify problems and areas for improvement, visiting buildings to identify issues with field clinicians, educating clinicians to correct documentation, reviewing patient charts and orders, and ensuring compliance with home care regulations. prior experience in home care and case management, with a valid NYS RN license and knowledge of Medicare and NYS regulations, is required; case management and clinical experience are preferred.
Required Qualifications
- Valid NYS RN License.
- 5+ years of Home Care experience (Required)
- Knowledge in home care regulations- including Medicare and NYS regulations
- Case Management experience(Preferred)
- Clinical Experience(Preferred)
