Job Summary
Coordinate interdisciplinary hospice services and conduct weekly medical assessments for patients and families in homecare settings within Kent County. Manage patient care plans, support primary caregivers with physical care, and provide hands-on nursing including patient lifting and turning while adhering to safety guidelines. Act as the primary point of contact to answer questions, explain expectations, and ensure comprehensive physical, emotional, and spiritual support. Requires a current Delaware RN license, valid driver's license, and CPR certification, with a preference for a Bachelor's degree and two years of nursing experience.
Required Qualifications
- Current Registered Nurse license to practice professional nursing in the State of Delaware
- Valid Driver's license that meets guidelines as outlined in the Policy
- Graduate of an approved school of nursing
- CPR certification
- Minimum of two years of nursing
- Must be able to bend, squat, drive for extended periods of time, lift and turn patients; lift 30 to 60 lbs, following industry standards and guidelines for safety and correct ergonomic practices
- Employees involved in patient care may be exposed to blood, body tissue and fluids regularly and are expected to follow all health care industry safety guidelines indicated in our procedures manual
- Active participant in coverage of all shifts as needed
