Job Summary
Provide strategic leadership and operational oversight for Case Management, Discharge Planning, Social Work, and Access Control functions. Coordinate standardized workflows to optimize patient flow, reduce length of stay, and improve clinical outcomes. Manage departmental budget, staffing, and performance while ensuring timely pre-certification for post-acute services to minimize financial risk. Oversee data analysis of key performance indicators including avoidable days and resource utilization. Collaborate with physicians and senior administration to implement clinical pathways and maintain accountability for acute inpatient Social Work functions. This role requires an RN with acute hospital experience, an MSN preferred, and a Director-level background in an acute hospital setting.
Required Qualifications
- Registered Nurse (RN) with acute hospital nursing experience
- Bachelor Degree in Nursing
- Director of Case Management in an Acute Hospital Setting
Desired Qualifications
- Masters in Nursing (MSN)
- Certification in Case Management (CCM/ACM) preferred within 2 years of hire
