Job Summary
RN Director of Case Management oversees Case Management, Discharge Planning, Social Work, and Access Control; provides strategic leadership and operational oversight, coordinates standardized workflows to support Care Coordination, and collaborates with physicians, nursing leadership, and senior administration to optimize patient flow, reduce length of stay, and improve outcomes. Responsible for timely pre-certification for post-acute services, minimizing financial risk, identifying opportunities for clinical process improvement, implementing clinical pathways, managing departmental budget and staffing to maximize productivity and efficiency, and overseeing data collection, analysis, and reporting of KPIs including avoidable days, LOS, and Resource Utilization. Maintains accountability for acute inpatient Social Work functions and participates in interdisciplinary team activities.
Required Qualifications
- Registered Nurse (RN) with acute hospital nursing experience required
- Bachelor Degree in Nursing required
- Director of Case Management in an Acute Hospital Setting required
