Job Summary
Coordinate care, assess, and triage patients in the ambulatory setting while managing medication and reviewing medical records. Monitor diagnostic results, track patient care progress, and determine appropriate scheduling protocols as requested by providers. Participate in quality reviews and work alongside a team of RN care coordinators to deliver evidence-based care. Utilize electronic health records and heavy phone use to support complex patient needs, requiring strong time management and independent decision-making skills.
Required Qualifications

- Requires ability to triage and assess complex patient symptoms/condition
- Requires excellent time management skills
- Heavy phone use and ability to use technology essential to position's success (EPIC)
- Must be able to work independently
- Ability to use the senses of seeing (including color vision ability), hearing, touch, and smell to make correct judgments regarding patient conditions and safely administer care
- Candidate must be detail-oriented and able to work well with electronic health record
- 1-2 years of nursing experience inpatient with a complex medical focus, or in ambulatory or community-based setting working with chronically ill or medically complex patients
- Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date
- Associate Degree in Nursing from an accredited program 30 days before the start date
- Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
- Current, unencumbered Oregon State Registered Nurse License
- WA license within 6 months
- BLS from AHA required
- Must be able to perform the essential functions of the position with or without accommodation
Desired Qualifications
- Experience in the field of surgery / GI medicine in an Ambulatory clinical setting is preferred
