RN Diabetes Case Manager & Educator

RN Diabetes Case Manager & Educator responsible for delivering a formal diabetes education program, providing education and case management to patients with diabetes, prediabetes, and gestational diabetes, maintaining diabetes registries, ensuring annual exams and education per IHS Standards of Care, coordinating prevention goals with the Consortium Diabetes team, supporting audit activities, and educating providers and staff on standards of care. Must model the Seven Standards of Excellence, assist with RPMS tools and audits, and respond to inquiries from clinical and non-clinical staff and patients. Education and licensure requirements include a current Alaska RN license and CDE (or eligible within 2 years); clinical competency assessment due within 3 months of hire and every three years. 3+ years general nursing or dietetic experience (preferred) and 1+ year diabetes care experience (preferred).
