Job Summary
RN Diabetes Case Manager and Educator responsible for maintaining a formal comprehensive diabetes education program across a multidisciplinary team, including medical providers and health educators. Responsibilities include managing diabetes registries, ensuring annual exams and education per IHS Standards of Care for Adults with Type 2 Diabetes, coordinating prevention goals with the SEARHC Diabetes team, leading education and training for providers and staff, facilitating diabetes self-management education, and ensuring high-quality care through audits and data reporting. Requires Alaska RN license and Diabetic Educator certification or eligibility; strong emphasis on patient education, care coordination, and culturally informed health promotion within Southeast Alaska.
Required Qualifications
- A valid, current RN license in the state of Alaska
- Certification as a Diabetic Educator OR eligible to apply for certification within 2 years
- Clinical Competency required within 3 months of hire and every three years
