RN Coordinator of Care Peds

RN Coordinator of Care (Peds) will manage all aspects of patient care related to home services as a Visiting Nurse. Responsibilities include conducting home visits, completing required documentation, and communicating with the PCP. This is a full-time role (Monday–Friday) with benefits such as sign-on bonus, tuition reimbursement, 401K matching, vacation, personal days, sick days, and monthly productivity bonuses. Office and field orientation provided; company tablet and technical support available. Requires NYS RN License, valid driver’s license, and ability to work independently with strong clinical skills and basic office software knowledge.
