RN COMPLEX CASE MANAGER

Job Summary
RN Complex Case Manager coordinates the care of high-risk patients in an ambulatory setting, focusing on planning, assessment, monitoring and evaluation of comprehensive health care needs in collaboration with patients/families/caregivers. Requires NV RN licensure, CPR/BLS, and 3+ years of clinical practice with experience in complex case management; proficient with EHRs and HIPAA compliance; works with an interdisciplinary team to develop and implement individualized care plans, improve patient outcomes, and optimize resource utilization.
Required Qualifications
- Graduate of an accredited nursing school required
- Licensure as a registered nurse in the state of Nevada required
- Current CPR or BLS certification required
- Minimum three years of experience in a clinical practice (ER, ICU)
- Familiarity with Electronic Health Record systems required
- Minimum two years experience in complex case management required
