Job Summary
Conduct thorough assessments to determine member health objectives and obstacles, then develop and implement individualized care plans in collaboration with the care team. Act as an advocate, resource, and educator for members and families while leveraging chronic disease management concepts to support self-management skills. Coordinate care transitions and ensure continuity across providers, services, and settings, adhering to relevant NCQA standards. Full-time hours from 8 am to 5 pm with no weekends or holidays. Compensation ranges from $72,500 to $90,600. An experienced team provides support for training and complex cases.
Required Qualifications
- Active WI RN or compact state license
- Bachelor or Associates Degree in Nursing
- 3 + years in clinical nursing, case management, or care coordination
- Strong critical thinking and problem-solving abilities
- Excellent communication and interpersonal skills to engage diverse populations
- Proficiency with electronic health records (EHR) and care management systems
- Knowledge of and skills in complex care management using CCM, CMSA, and/or ACM models
- Cultural competence and sensitivity to work with individuals from varied backgrounds
- Willingness to obtain Case Management (CCM) certification within one year
