Job Summary
We are seeking a compassionate Registered Nurse to join our home care team as a Community-Based Case Manager. Responsibilities include supervising and mentoring nursing staff, conducting in-home care assessments, developing care plans, and coordinating with multidisciplinary teams. Candidates must possess a current RN license in Maryland, a minimum of 1 year of nursing experience, strong knowledge of healthcare regulations, excellent leadership and communication skills, and a commitment to patient-centered care.
Required Qualifications
- Current RN license in the state of Maryland
- Minimum of 1 years of nursing experience
- Strong knowledge of healthcare regulations and best practices

Desired Qualifications
- Supervisory experience preferred
- CPR certification and other relevant certifications preferred
