Job Summary
Provide direct nursing care to patients across inpatient rehabilitation, home health, and hospice settings. Monitor patient progress, administer medications as prescribed, and collaborate with multidisciplinary health teams to support recovery and optimize health outcomes. Conduct comprehensive assessments, develop and implement individualized care plans, document patient status accurately, and ensure adherence to safety, quality standards and evidence-based practices. Demonstrate strong clinical skills in nursing assessment, care coordination, and patient and family education to support functional improvement and quality of life.
Required Qualifications
- Registered Nurse (RN) license in South Carolina or eligibility
- Current nursing license/credential appropriate for SC practice
- BLS (Basic Life Support) certification
- ACLS preferred/not specified in posting
- Experience in inpatient rehabilitation, home health, or hospice environments as applicable to role
