RN Clinical Team Specialist

RN Clinical Team Specialist responsibilities include overseeing patient care services, coordinating referrals and ensuring ongoing assessment of patient needs; reviewing and approving plans of care while ensuring clinical appropriateness; conducting OASIS reviews and ensuring timely visits; monitoring documentation for accuracy/compliance with licensing regulations; contributing to strategic planning and QAPI activities; performing additional duties as assigned; and maintaining proficiency with enterprise applications (Homecare Homebase, Workday) and Microsoft Office. The role requires an Associate degree in nursing, RN licensure, 2 years of home care experience (home health or hospice), travel as needed, strong documentation and compliance skills, and physical ability to lift up to 50 pounds.
