Job Summary
Provide clinical supervision, training, and evaluation of RN Case Management and Care Management staff within the Multipurpose Senior Services Program (MSSP). Responsible for ensuring compliance with regulatory requirements, conducting audits, and facilitating care plan conferences. Develop and implement educational programs for staff while maintaining collaborative relationships with community organizations. Requires an RN, BSN with 3-5 years of relevant experience, and strong leadership and communication skills.
Required Qualifications
- RN, BSN
- 3-5 years experience working with frail elderly, adolescents, disabled adults and children
- Supervisory experience preferred
- Exceptional communication, presentation skills, and time management
- Detail oriented and well organized
- Demonstrates case management skills and experience in the community health care delivery system
- Ability to supervise care management staff and provide coaching/mentoring to staff
- Good problem-solving skills and the ability to prioritize and multi-task
- Computer literacy required (Excel, Word)
- Ability to navigate multiple platforms and EMRs
