RN Clinical Supervisor - Hybrid Schedule

RN Clinical Supervisor leading internal case management and overseeing a caseload of medically fragile patients, performing Start of Care assessments and re-evaluations to transition facilities to home care, reviewing physicians’ orders, updating care plans and documentation, and assisting in interviewing/training/validating new clinical staff. Will mentor and guide field staff to deliver safe, high-quality in-home nursing care, collaborate with operations and business development, ensure compliance with applicable policies and procedures, and travel to patients’ homes within the designated territory. Position combines clinical expertise with administrative responsibilities in a full-time role with on-call rotations; hybrid work arrangement varies by branch need.
