RN Clinical Supervisor - Hybrid Schedule

RN Clinical Supervisor leading internal case management and staff development for in-home skilled nursing care. Responsibilities include performing Start of Care assessments and re-evaluations to transition patients from facilities to home care, reviewing physicians’ orders, updating care plans and documentation, interviewing and training new clinical staff, validating nurse competencies, mentoring field staff to ensure safe, high-quality nursing care, collaborating with operations and business development, and ensuring compliance with all policies and procedures. Requires traveling to patients’ homes within the designated territory, conducting on-call rotations, and maintaining an active RN license in practice; CPR certification is required. This leadership role focuses on managing a caseload of medically fragile patients and guiding field staff to deliver exceptional care in a dynamic, compliant environment.
