Job Summary
Review new patient referrals to assign hospice staff, support clinical teams through home visits and case reviews, and lead team case conferences for coordinated care. Develop patient care plans, conduct regular chart audits for quality improvement, and assist in policy development while mentoring staff through hiring and continuing education programs. Engage in community outreach to promote the hospice's mission and uphold professional care standards. Monday-Friday 8am-5pm schedule in New Braunfels, TX.
Required Qualifications
- Registered Nurse with current, unencumbered licensure to practice professional nursing in the state of operation
- Previous experience in hospice or home care setting
- Current CPR certification, maintained in accordance with organizational policy
- Must be a licensed driver with an automobile that is insured per state and/or organizational requirements and is in good working order
- Proven ability to work effectively within an interdisciplinary team setting
- Demonstrates excellent observation, nursing judgment, and communication skills—both verbal and written
- Understands the hospice philosophy and the emotional, clinical, and ethical considerations surrounding end-of-life care
- Knowledge of pain and symptom management principles
- Adheres to accepted professional standards and practices
- Commitment to clinical excellence and thorough documentation
Desired Qualifications
- Graduate of an accredited school of nursing; BSN preferred
- Minimum of two (2) years of management or supervisory experience
