Job Summary
Coordinate home health services to ensure delivery meets acceptable standards and company procedures. Review and approve patient information submitted during start of care, recertification, resumption of care, or evaluation visits. Facilitate relationships between physicians, referral sources, patients, caregivers, and employees. Maintain a valid state driver's license, automobile liability insurance, and dependable transportation in good working condition. Must be licensed in the state of practice with at least two years of nursing experience and one year in home health or hospice.
Required Qualifications
- Graduate of an approved school of professional nursing
- Licensed in the state in which they currently practice, or in accordance with the board of nursing rules for nurse licensure compact for the state in which they practice
- At least two years of nursing experience
- One year experience in home health or hospice
- Demonstrated knowledge and understanding of the federal, state, and local laws and regulatory guidelines that govern a home care operation
- Basic demonstrated technology skills, including operation of a mobile device
- Valid state driver license
- Automobile liability insurance as required by law
- Dependable transportation in good working condition
- Ability to safely drive an automobile in all types of weather conditions
