RN Clinical Nurse II - CPSU

Provides competent clinical nursing care in a home health setting in accordance with professional standards, coordinating and delivering patient care using the nursing process. Responsibilities include education of patient/family/caregivers, evaluation and planning of care with an interdisciplinary team, ensuring timely communication to promote continuity of care, and leadership to develop professional growth and optimize outcomes. Demonstrates competence in caring for patients with complex problems, utilizes organizational policies and procedures, prioritizes care, collaborates with the patient/family to update the plan of care, and applies cultural considerations in planning care. Familiar with discharge planning and promoting cost-effective care, with emphasis on patient safety, education, and advocacy for patients and families.
