RN Clinical Manager

Clinical Manager responsible for the implementation, delivery, coordination, and supervision of home health services. Serves as a clinical and managerial resource for problem solving and daily delivery of quality patient care. Oversees assessment, plan of care, admissions/screening, and coordination across a multi-disciplinary team (RN, LPN, Therapists, Social Workers, CNA). Ensures compliance with Medicare, State regulations and CHAP accreditation; drives quality assurance and performance improvement, staff development, on-site field supervision, and liaison among Intake, Business Development, and Patient Care Coordinators. Represents the agency in professional settings, maintains patient confidentiality (HIPAA), and supports accreditation and reimbursement processes.
