Job Summary
Assess patients and families to determine home care needs, develop care plans with therapeutic and preventive actions, and initiate preventive nursing procedures. Coordinate patient care as a case manager, counsel patients and families, and provide health care instructions while managing discharge planning. Generate Medicare referrals for hospice and home care by building relationships with physicians, social workers, and facility directors. Conduct sales calls, perform market analysis, and develop sales strategies to expand business opportunities with hospitals and skilled nursing facilities. Adhere to CRM tools and anti-kickback laws while participating in on-call duties and supervising home health aides.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in state
- CPR certification
- Bachelor's degree
- One (1) year of home health care experience
- One (1) to two (2) years of recent acute care experience in an institutional setting
- Excellent observation skills
- Verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Prolonged or considerable walking or standing
- Ability to lift, position or transfer patients
- Ability to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling or crouching
- Visual acuity and hearing to perform required nursing skills
- Must be a licensed driver
- Automobile insured in accordance with state/or organization requirements
