Job Summary
Receive and respond to referrals from hospitals, physicians, and case managers while assessing patient eligibility through clinical evaluations and medical record reviews. Educate patients and families on hospice care protocols, then complete necessary documentation to coordinate intake and admission processes. Make daily visits to facilities to maintain visibility with discharge planners and physicians, ensuring seamless handoffs and rapid responses to needs. Build strong relationships with referral partners to increase awareness of hospice services.
Required Qualifications
- Licensed Nurse
- three to five years of hospice experience
- Current RN license to practice nursing in the state of employment
- CPR certification
- Valid driver's license
- automobile insurance
- Minimum of three years of hospice experience
Desired Qualifications
- BSN
- five or more years of hospice experience
- Prior experience as a discharge planner
- Prior experience as a case manager
- Prior experience as an educator
- Strong knowledge of hospice care
- Familiarity with community resources and referral networks
- Excellent patient assessment and documentation skills
- Ability to supervise staff
- Ability to coordinate services
- Ability to manage programs effectively
