Job Summary
Conduct person-centered in-home nursing assessments for the Medicaid CFCS/PCS program, evaluating functional abilities and needs for ADL and IADL assistance. Perform annual reviews and ad-hoc assessments when condition changes, coordinating care with provider agencies and ensuring service delivery aligns with State of Montana DPHHS policies. Practice cultural humility, utilize evidence-based interventions, and empower members to remain independent in their homes through active listening and planning. This full-time remote role requires residing in Missoula or Ravalli County, traveling by personal vehicle for assessments, and maintaining reliable home internet. Candidates must hold a current Montana RN license with experience in home health, hospice, or long-term care preferred. Work independently within a flexible 30-40-hour schedule with full wage coverage for travel time and mileage reimbursement.
