Job Summary
RN Clinical Coordinator to lead outpatient dialysis care for patients with end-stage renal disease. Responsibilities include coordinating patient care plans and monitoring outcomes, supervising clinical staff (including PCTs), ensuring safe, compassionate dialysis delivery, building long-term relationships with patients and families, and working in a fast-paced, team-oriented environment. Requires active RN license, CPR, and 18+ months RN experience with dialysis exposure; ADN required with BSN preferred; ICU/ER/Med-Surg experience encouraged; flexibility to cover weekends/holidays; leadership readiness and ability to supervise staff.
Required Qualifications
- Current RN license
- CPR certification
- 18+ months RN experience including 6+ months dialysis
- ADN required; BSN preferred
- Charge RN readiness approval required
- CNN/CDN certification a plus
- Basic computer skills (MS Word, Outlook)
- Flexible schedule including weekends and holidays
